How to Apply

Before filling out an application we encourage you to review LTC Basics, Cost of Care and Plan Details to learn more about different plan types and optional benefits available to you. After you have reviewed this information, go to Get a Quote for a plan that appeals to you using our premium calculator tool. If you haven’t read the entire Application Kit, which contains an Information Guide, long-term care customer information and Outline of Coverage, please download the kit or request one by contacting us at or calling (800) 908-9119. Once you’ve reviewed the kit, you’re ready to fill out an application.

You have three ways to apply for coverage:

1. Complete an application online.

2. Download an application kit to your computer, print and mail to us your completed application.

3. Request an application kit be mailed to you and mail to us your completed application using the one included in the kit.

Note: each person applying for coverage must complete a separate application.

Your application will be returned if any section requiring signature is not signed.

If you attach additional information to your application, be sure to include your name and Social Security number on all attachments.

Guidance for Existing Participants

If you are an existing participant in CalPERS Long-Term Care, you are welcome to apply for the new Comprehensive or Partnership plans if you currently meet eligibility requirements. If you pass underwriting and accept the new coverage, your premium will be based on your age at the time of application for the new coverage and your existing coverage will be terminated. You will not receive a premium credit for your prior CalPERS Long-Term Care coverage. Please do not cancel your current coverage until you have passed underwriting for the new coverage. We recommend seeking advice from a financial advisor when considering a plan change.

Notice of Privacy Practices

We are committed to protecting the privacy of both the applicants for CalPERS Long-Term Care coverage and existing CalPERS Long-Term Care participants. The manner in which we handle Protected Health Information is detailed in our Notice of Privacy Practices.

The Notice of Privacy Practices defines how we may use and disclose Protected Health Information for: treatment, payment or healthcare operations, and other purposes required by law. LTCG serves as CalPERS third party administrator for the CalPERS Long-Term Care Program. As such, they are considered a Business Associate under the Health Information Technology for Economic and Clinical Health Act. LTCG is accountable for complying with the Health Information Portability and Accountability Act, which regulates the use and disclosure of personal health information. LTCG will honor the appropriate requirements under CalPERS Notice of Privacy Practices regarding use and disclosure of Protected Health Information.
